2df07ddc25 Fix implicit leaks of imports throughout libraries 
Also touch up use of 'pub' and move some tests around so the tested functions
don't have to be 'pub'

08b6057538 Macros now leave scope 
Macro scope is now delimited by function, block, and module boundaries,
except for modules that are marked with #[macro_escape], which allows
macros to escape.

319eeb1c79 rename map -> oldmap and mark it as deprecated 
LinearMap is quite a bit faster, and is fully owned/sendable without
requiring copies. The older std::map also doesn't use explicit self and
relies on mutable fields.

1bede1f5e0 Replace several common macros of the form #m[...] with m!(...) 
This commit replaces nearly all remaining uses of #fmt, #debug, #error,
