Preliminary support for labeled break/continue for loops

This patch adds preliminary middle-end support (liveness and trans)
for breaks and `loop`s to `loop` constructs that have labels.
while and for loops can't have labels yet.

Don't check impl ty params for equality with trait ty params
...
This was too restrictive. We need to check the number of ty params,
and that the bounds are equal, but otherwise require_same_types does the job.

Most could use the each method, but because of the hack used to
disambiguate old- and new-style loops, some had to use vec::each.
(This hack will go away soon.)

In typestate, consider infinite loops w/ conts to be infinite
...
If a loop { } contains a cont, that doesn't affect whether the entire
loop diverges. Only breaks affect that. Fix that in typestate.

Rename last to last_opt, last_unsafe to last
...
As per discussion on IRC. I am about to file an RFC for further
discussion about the more general issue of whether to enforce
invariants through types, typestate, or dynamic checks, but for now,
removing the misleading name "last_unsafe".
