fix: Fix formatting requests hanging when r-a is still starting
...
The reason for that was that we were calculating the crate defmaps
of the file we are saving by accident causing us to get stuck waiting
on their expensive computation, while we only need the relevant crate
id.

Don't expose SyntaxKind from IDE API
...
SyntaxKind is somewhat of an internal type, but IDE is using it to
basically specify an icon. Let's have a dedicated entity for this
instead.

Move ide::AnalysisChange -> base_db::Change
...
This seems like a better factoring logically; ideally, clients shouldn't touch
`set_` methods of the database directly. Additionally, I think this
should remove the unfortunate duplication in fixture code.
