These commits fix bugs related to identically named statics in functions of implementations in various situations. The commit messages have most of the information about what bugs are being fixed and why.
As a bonus, while I was messing around with name mangling, I improved the backtraces we'll get in gdb by removing `__extensions__` for the trait/type being implemented and by adding the method name as well. Yay!
As with the previous commit, this is targeted at removing the possibility of
collisions between statics. The main use case here is when there's a
type-parametric function with an inner static that's compiled as a library.
Before this commit, any impl would generate a path item of "__extensions__".
This changes this identifier to be a "pretty name", which is either the last
element of the path of the trait implemented or the last element of the type's
path that's being implemented. That doesn't quite cut it though, so the (trait,
type) pair is hashed and again used to append information to the symbol.
Essentially, __extensions__ was removed for something nicer for debugging, and
then some more information was added to symbol name by including a hash of the
trait being implemented and type it's being implemented for. This should prevent
colliding names for inner statics in regular functions with similar names.
Whenever a generic function was encountered, only the top-level items were
recursed upon, even though the function could contain items inside blocks or
nested inside of other expressions. This fixes the existing code from traversing
just the top level items to using a Visitor to deeply recurse and find any items
which need to be translated.
This was uncovered when building code with --lib, because the encode_symbol
function would panic once it found that an item hadn't been translated.

This requires changes to method search and to codegen. We now emit a
vtable for objects that includes methods from all supertraits.
Closes#4100.
Also, actually populate the cache for vtables, and also key it by type
so that it actually works.
.with_c_str() is a replacement for the old .as_c_str(), to avoid
unnecessary boilerplate.
Replace all usages of .to_c_str().with_ref() with .with_c_str().
Currently we pass all "self" arguments by reference, for the pointer
variants this means that we end up with double indirection which causes
a unnecessary performance hit.
The fix itself is pretty straight-forward and just means that "self"
needs to be handled like any other argument, except for by-value "self"
which still needs to be passed by reference. This is because
non-pointer types can't just be stuffed into the environment slot which
is used to pass "self".
What made things tricky is that there was also a bug in the typechecker
where the method map entries are created. For type impls, that stored
the base type instead of the actual self-type in the method map, e.g.
Foo instead of &Foo for &self. That worked with pass-by-reference, but
fails with pass-by-value which needs the real type.
Code that makes use of methods seems to be about 10% faster with this
change. Also, build times are reduced by about 4%.

The code that tried to revoke the cleanup for the self argument tried
to use "llself" to do so, but the cleanup might actually be registered
with a different ValueRef due to e.g. casting. Currently, this is
worked around by early revocation of the cleanup for self in
trans_self_arg.
To handle this correctly, we have to put the ValueRef for the cleanup
into the MethodData, so trans_call_inner can use it to revoke the
cleanup when it's actually supposed to.
"self" is always passed as an opaque box, so there's no point in using
the concrete self type when translating the argument. All it does it
causing the value to be casted back to an opaque box right away.
This removes the `namegen` thunk that was in `common.rs`. I also take the opportunity to refactor a few uses where we had a `str -> ident -> str` chain that seemed somewhat redundant to me.
Also cleans up some warnings that made their way in already.
