Support explicit discriminant numbers on tag variants.

Addresses issue #1393 .
For now disallow disr. values unless all variants use nullary
contractors (i.e. "enum-like").
Disr. values are now encoded in the crate metadata, but only when it
will differ from the inferred value based on the order.

Adjust function signatures to allow for vecs being immediate

Some code was relying on vectors being implicitly by-reference (as
non-immediate value). This adds the necessary &&-sigils.

Encode/decode a crate's externel dependencies. Issue #632

The encoding is very simple right now, just the crate name. Ultimately this
won't be enough for our versioning needs, but it should fill our immediate
need of being able to correlate encoded crate numbers to actual crates.
