Revert map.each to something which takes two parameters
...
rather than a tuple. The current setup iterates over
`BaseIter<(&'self K, &'self V)>` where 'self is a lifetime declared
*in the each method*. You can't place such a type in
the impl declaration. The compiler currently allows it,
but this will not be legal under #5656 and I'm pretty sure
it's not sound now.

Convert all uses of vec::slice to vec::view Issue #3869
Rename const_view to const_slice
Renamed mut_view to mut_slice
Fix windows build error. `buf` is borrowed by the call to
`as_mut_buf()` and so we must invoke `slice()` outside of that
call.

rename map -> oldmap and mark it as deprecated
...
LinearMap is quite a bit faster, and is fully owned/sendable without
requiring copies. The older std::map also doesn't use explicit self and
relies on mutable fields.

Add improvements to insert_with_key
...
This commit adds a lower-level implementation of the generic
`insert_with_key` which I expect to be faster. Now insert could be
defined with insert_with_key, too, although I'm not sure we want to do that.
This also clarifies the tests a bit and adds an `insert_with` function.
