bdc182cc41 Use static string with fail!() and remove fail!(fmt!()) 
fail!() used to require owned strings but can handle static strings
now. Also, it can pass its arguments to fmt!() on its own, no need for
the caller to call fmt!() itself.

852619d5d7 Remove ++ mode from the compiler (it is parsed as + mode) 
and obsolete `-` mode altogether (it *was* parsed as `+` mode).

b18320446e Move over to calling ptr::addr_of 
Everything should now call ptr::addr_of instead of
ptr::p2::addr_of. Only the pipes macro code when compiled
by stage0 will call ptr::p2::addr_of. Needs a snapshot to get
rid of that.
