efca32e23a Rename some OwnerId fields. 
spastorino noticed some silly expressions like `item_id.def_id.def_id`.

This commit renames several `def_id: OwnerId` fields as `owner_id`, so
those expressions become `item_id.owner_id.def_id`.

`item_id.owner_id.local_def_id` would be even clearer, but the use of
`def_id` for values of type `LocalDefId` is *very* widespread, so I left
that alone.

9136096629 rustc: Parameterize ty::Visibility over used ID 
It allows using `LocalDefId` instead of `DefId` when possible, and also encode cheaper `Visibility<DefIndex>` into metadata.

e110231260 Improve AdtDef interning. 
This commit makes `AdtDef` use `Interned`. Much the commit is tedious
changes to introduce getter functions. The interesting changes are in
`compiler/rustc_middle/src/ty/adt.rs`.

c864e18678 Remove some last remants of {push,pop}_unsafe! 
These macros have already been removed, but there was still some code
handling these macros. That code is now removed.

dfb41f4797 Separate out a hir::Impl struct 
This makes it possible to pass the `Impl` directly to functions, instead
of having to pass each of the many fields one at a time. It also
simplifies matches in many cases.
